Abstract
Agresivnost je prijeteće ponašanje koje rezultira psihičkom, fizičkom ili materijalnom štetom. Može biti usmjerena prema samome sebi, prema drugim osobama ili prema objektima/stvarima. Agresija se može izraziti fizički, verbalno ili čak izostajanjem aktivnosti, odnosno zanemarivanjem. Agresija u pojedincima ovisi o raznim rizičnim faktorima i čimbenicima koji utječu na dozu agresivnosti te samim time i na agresivnu eskalaciju. Kako i u društvu i u populaciji tako se agresivne osobe nalaze i na psihijatrijskim odjelima. Postoje razna psihička stanja koja dovode do agresivnog akta. Shizofreni bolesnici mogu agresivno nastupati kao i osobe koje se nalaze u alkoholno predoziranom stanju ili osobe koje su se predozirale opiodnim tvarima. Bilo koji bolesnik koji pokušava ozlijediti sebe, drugu osobu ili koji pokušava nanijeti veliku materijalnu štetu nastoji se zbrinuti neprisiljavajućim ili prisiljavajućim metodama. Kao neprisiljavajuće metode navode se postupci deeskalacije koji se baziraju na komunikaciji s bolesnikom. Pomoću komunikacijskih vještina bolesnik se nastoji smiriti riječima te prisutnom atmosferom tijekom komunikacije. Prisiljavajuće metode se koriste samo onda kada neprisiljavajuće metode nisu uspjele. U prisiljavajuće metode ubrajamo farmakološku terapiju, izolaciju te fizičko sputavanje. Farmakološka terapija obuhvaća umirivanje bolesnika putem davanja terapije, odnosno lijekova. Izolacija je postupak odvajanja agresivnog bolesnika, od ostalih bolesnika s odjela, u posebno uređenu sobu u kojoj boravi sve dok liječnik ne procjeni da se bolesnik može vratiti u sobu u kojoj boravi s ostalim bolesnicima. Fizičko sputavanje je zadnja faza prisiljavajućih metoda, ona obuhvaća humano fizičko sputavanje bolesnika s ciljem da se bolesnik umiri i prestane biti agresivan. Fiksiraju se ekstremiteti, glava i prsa. Bolesnik se odvezuje samo i isključivo kada liječnik putem psihofizičke procjene odluči da bolesnik više ne treba biti fiksiran. Edukacija je veoma bitan proces u ovakvim slučajevima. Educiranjem osoblja se uklanja njihov mogući strah ili nesigurnost i samim time se povećava sigurnost svih osoba koje borave na odjelu na kojem može doći do agresivne eskalacije.
Abstract (english)
Aggressiveness is a threatening behavior that results in mental, physical or material harm. It can be directed towards oneself, towards other persons or towards objects / things. Aggression can be expressed physically, verbally or even by neglect. Aggression in individuals depends on various risk factors and factors that influence the dose of aggression and therefore aggressive escalation. As in society and in the population, so do aggressive individuals find themselves in psychiatric wards. There are various psychic conditions that lead to an aggressive act. Schizophrenic patients can act aggressively as well as people who are in an alcohol overdose or people who have been overdosed on opioid substances. Any patient who tries to injure himself, another person, or who tries to cause great material damage shall endeavor to be taken care of by non-punitive or coercive methods. Deescalation procedures based on communication with the patient and using communication skills through which the patient seeks to calm down with words and the atmosphere during communication are cited as non-striking methods. Coercive methods are only used when non-coercive methods have failed. Forcing methods include pharmacological therapy, isolation and physical restraint. Pharmacological therapy involves the appeasement of the patient by administering therapy or medication. Isolation is the process of separating an aggressive patient from other patients from the ward into a specially arranged room where he or she resides until the physician estimates that the patient can return to the room where he or she resides with other patients. Physical restraint is the last phase of coercive methods, it involves the humane physical restraint of the patient with the aim of calming the patient and ceasing to be aggressive. The extremities, head and chest are fixed. The patient is unbound only when the physician determines by psychophysical assessment that the patient no longer needs to be fixed. Education is a very important process in such cases. Educating staff eliminates their possible fear or insecurity and increases the security of all people in the department where aggressive escalation can occur.
