Abstract
UVOD: Akutno krvarenje iz gornjeg gastrointestinalnog trakta (GIT) je jedno od najčešćih hitnih stanja u gastroenterologiji. Od velike je važnosti prepoznati potencijalne etiološke čimbenike za krvarenje iz gornjeg GIT-a i razumjeti terapeutske modalitete da bi se postigla hemostaza kod pacijenta. Moderna endoskopija ezofagogastroduodenoskopija (EGD) i njene opcije intervencija su trenutno neosporna metoda i temelj izbora za dijagnozu i liječenje krvarenja iz gornjeg GIT-a, a preporuča se učiniti istu u roku od 24 sata. Stopa mortaliteta iznosi od 6%-13% (1). CILJ ISTRAŽIVANJA: Utvrditi prevalenciju krvarenja iz gornjeg GIT-a tijekom 2015. godine u Općoj bolnici Zadar, te prikazati ulogu medicinske sestre od pripreme za endoskopsku pretragu, prijema pacijenta na odjel, do prevencije komplikacija krvarenja iz gornjeg GIT-a, kao i samu edukaciju pacijenta. ISPITANICI I METODE: U istraživanje su uključeni pacijenti s medicinskom dijagnozom krvarenja iz gornjeg GIT-a hospitalizirani na Odsjeku internističke intenzivne skrbi, Službe za interne bolesti, OB Zadar u periodu od 01.01.2015. do 31.12.2015. godine. Istraživanjem je uključeno 120 pacijenata, od čega 85 muškaraca i 35 žena. REZULTATI: Pojavnost krvarenja iz gornjeg GIT-a je češća kod muškaraca nego kod žena i to u dobi od 60-69 godina za muškarce, a kod žena 70 do 89 godina. Kardiovaskularne bolesti zauzimaju udio od 69%, a lijekove koji povećavaju rizik od krvarenja je uzimalo 38% ispitanika. Konzumacija alkohola i navika pušenja je češća kod muškaraca. Melena i hematemeza su najčešći specifični znak uz osjećaj slabosti i mučnine. Ulkus želuca pojavljuje se kod 27%, ulkus duodenuma kod 25%, te varikoziteti jednjaka kod 19% ispitanika. Endoskopija s metodama hemostaze je primijenjena kod 73% ispitanika. Najčešći stupanj klasifikacije ulkusa po Foresstu je bio FIB (slijevajuće vensko krvarenje) kod 36% ispitanika. Recidiv krvarenja iz gornjeg GIT-a iznosi 10%. Udio mortaliteta iznosi 6% za oba spola. ZAKLJUČAK: Na pojavnost krvarenja iz gornjeg GIT-a utječe višestruko rizičnih faktora, poput spola, dobi, komorbiditeta, uzimanje antikoagulantne terapije i antitrombotske terapije, te nestereoidnih antireumatika, konzumacije alkohola i pušenje. Uloga medicinske sestre je educirati pacijenta o smanjenu rizičnih faktora i prevenciji komplikacija, te tako potaknuti pacijenta na uključivanje o planu promjene životnih navika.

INTRODUCTION: Acute upper GI bleeding is one of the most common emergencies in gastroenterology. It is important to recognize potential etiological factors for upper GI bleeding and to understand the therapeutic modalities to achieve hemostasis at the patient. Modern endoscopy such as esophagogastroduodenoscopy (EGD) and its intervention options are currently the indisputable methods and the base of choice for the diagnosis and treatment of upper GI bleeding and it is recommended to do the same within 24 hours. Mortality rate ranges from 6%-13% (1). OBJECTIVES: To determine the prevalence of upper GI bleeding during 2015. in General hospital Zadar, and show the role of a nurse in the preparation for EGD, admission the patient to the hospital, prevention complications of upper GI bleeding from and to very patient education. PARTICIPANTS AND METHODS: The study involved patients with a medical diagnosis upper GI bleeding treated at the Department of internal intensive care, Department of internal medicine, General hospital Zadar for a period from 01.01.2015. to 31.12.2015. The study included 120 patients, of which 85 men and 35 women. RESULTS: The incidence of the upper GI bleeding is more common in men than in women aged 60-69 for men and women 70 to 89 years. Cardiovascular diseases occupy the pro-portion of 69%, and 38% of the subjects took drugs that increase the risk of bleeding. Alcohol consumption and smoking habit is more common in men. Melena and haematemesis are the most common specific sign with a symptoms such as weakness and nausea. Gastric ulcer occurs in 27%, duodenal ulcer in 25%, and esophageal varices in 19% of subjects. Endoscopy with its hemostasis methods has been applied in 73% of subjects. The most common ulcer level according to Foresst classification was FIB (flowing venous bleeding) with 36% of respondents. Recurrence of upper GI bleeding is 10%. Mortality rate is 6% for both sexes. CONCLUSION: Multiple risk factors effects on the incidence of upper GI bleeding from the such as gender, age, comorbidity, taking anticoagulant therapy and antiplatelet therapy, no steroidal anti-inflammatory drugs, alcohol and smoking. The role of a nurse is to educate patients about reducing risk factors and prevention of complications, and also encourage the patient to activate the lifestyle changes.
